Instructions:

1. Use this form for requesting massage therapy at your event. When done print the form and follow the instructions for submitting below.
2. If this is a recurring event submit a separate form each time event occurs.
3. Requests must be submitted 8 weeks in advance of the event date.
4. Submitting doesn’t mean your event has been approved or scheduled. You will receive a confirmation email as soon as your event is scheduled.

For the Ithaca campus you can send this form to the Clinic Coordinator via fax to 607-272-4271, or mail to Finger Lakes School of Massage, 1251 Trumansburg Road, Ithaca, NY 14850, attn: Clinic Coordinator.

For the Mount Kisco campus send the form to the Clinic Coordinator via fax to 914-241-7363, or mail to Finger Lakes School of Massage, 272 N. Bedford Rd., Mount Kisco, NY 10549, attn: Clinic Coordinator.
