Transcript & Diploma Request

INSTRUCTIONS & POLICIES:
1. Please print the request form using the link below. Complete all sections with the required information, read and sign the consent section. Your request will be delayed if you don't fill out the form completely.
2. We cannot accept requests via telephone or email.
3. Please include payment with the form and mail it to the campus you attended. Address the envelope as follows:
   Ithaca campus: Transcript Requests, FLSM, 1251 Trumansburg Road, Ithaca, NY 14850.
   Mt. Kisco campus: Transcript Requests, FLSM, 272 North Bedford Road, Mount Kisco, NY 10549
4. Please allow 3-5 business days to process your request. Your transcript or diploma will be sent via U.S. Mail (we cannot send out these documents via fax) within 3-5 business days from the request is received.
5. The request must be made by the FLSM graduate. Third party requests are not allowed. Also, the FLSM graduate is responsible for providing a correct and complete mailing address.
