2. Please include payment with the form and mail it to FLSM, 1251 Trumansburg Road, Ithaca, NY 14850, attention: Registrar.
3. Please allow 3-5 business days to process your request.
4. Your transcript or diploma will be sent via U.S. Mail (we cannot send out these documents via fax.)
5. The request must be made by the FLSM graduate. Third party requests are not allowed.
